Our office is located in the Regus Suite at 11811 N. Tatum Boulevard, Suite 3031, Phoenix, Arizona, 85028.

Please note that some of our locations do not have external signage, so please see the detailed instructions below for driving and parking directions, office policies and more.

OFFICE ACCESS

We are located at the corner of East Paradise and North Tatum within Paradise Village Parkway.

You will turn onto East Paradise and turn into the North End parking lot, which will be marked. There is free, uncovered, ground-floor parking available, including reserved accessible spots. You will enter the building and take the elevator to the third floor, then a left off of the elevator. 

When you arrive at the Regus Suite, you may need to call the number listed on the door to let the staff know that you have arrived. The front desk staff will then let you in and will notify your provider that you have arrived. 

LATE POLICY

If you will be more than 15 minutes late for your appointment, please contact our office as your appointment may need to be rescheduled. This will ensure the quality and timeliness of every patient's care.
