Our office is located in HQ at 601 Carlson Parkway, Suite 1050, Minnetonka, Minnesota, 55305.

driving & PARKING

When you arrive at 601 Carlson Parkway, you’ll be at a larger office building in a business park area. The entrance is clearly marked and easy to find. Parking is free and drop-off is right at the main entrance.

Office access

Go in through the main doors into the lobby area. From there, take the elevator up to the 10th floor. Once you exit the elevator, follow the signs to HQ Suite 1050 — suites on these floors are usually well labeled.

Please note: If the door is closed, please knock, then check in with reception and they’ll let your provider know you have arrived.

LATE POLICY

If you will be more than 15 minutes late for your appointment, please contact our office as your appointment may need to be rescheduled. This will ensure the quality and timeliness of every patient's care.
