Our office is located in Spaces at 500 W. Silver Spring Drive, Suite K200, Glendale, Wisconsin, 53217.

Please note that some of our locations do not have external signage, so please see the detailed instructions below for driving and parking directions, office policies and more.

driving & PARKING

Our office is located within the Bayshore Town Center in the Spaces suite, office K200.

Parking: There is a large surface parking lot, and it’s free. You can be dropped off right at the main entrance.

Public Transit: You can use bus routes along Silver Spring Drive

Driving: From I-43, take Silver Spring Dr exit, then head west. The plaza will be on your right.

Office access

Once inside, you can take the elevator to the second floor and find us in K200. If the door is closed, please knock. Reception will let your provider know you have arrived.

LATE POLICY

If you will be more than 15 minutes late for your appointment, please contact our office as your appointment may need to be rescheduled. This will ensure the quality and timeliness of every patient's care.
