DRIVING DIRECTIONS

We are located near the intersection of Gran Bay Parkway W. and Old St. Augustine Road. When you turn into our parking lot, there are two buildings and we are in the one on the right side, closest to Applebee’s. There is uncovered parking available with reserved accessible parking spots available. When you enter our building, you will take the elevator to the fourth floor. 

OFFICE ACCESS

At the Regus Suite, you may need to call the number listed on the door to let the staff know that you have arrived.

The front desk staff will then let you in and will notify your provider that you have arrived. 


LATE POLICY

If you will be more than 15 minutes late for your appointment, please contact our office as your appointment may need to be rescheduled. This will ensure the quality and timeliness of every patient's care.
