Our office is located in the Regus Suite at 841 Prudential Dr 12th Floor, Jacksonville, FL 32207.
Please note that some of our locations do not have external signage, so please see the detailed instructions below for driving and parking directions, office policies and more.

DRIVING DIRECTIONS
Our office is directly across from the Baptist Hospital. Park in the 841 parking garage on the first floor, which is reserved for guests/visitors. Note: this parking deck can cost up to $10.
If you are taking rideshare, you can be dropped off at the door. There are also accessible, reserved spots in the parking garage, as well.
OFFICE ACCESS
When you enter the 841 building, please take the first set of elevators to the right, and go to the 12th floor.
At the Regus Suite, you may need to call the number listed on the door to let the staff know that you have arrived.
The front desk staff will then let you in and will notify your provider that you have arrived.
LATE POLICY
If you will be more than 15 minutes late for your appointment, please contact our office as your appointment may need to be rescheduled. This will ensure the quality and timeliness of every patient's care.
